You’ve catalogued the warning signs — poor grades, behavior reports, and correspondence with teachers. You’ve had meetings with the school to discuss learning strategies. Yet, your child is still floundering.
It’s time to request an IEP evaluation from the school, but where do you start?
Get the ball rolling by sending a letter, modeled off of this sample, to your district’s head of special education, with a copy to your child’s principal.
Your school may have an online or print IEP or 504 application. You should include this, and any backup documentation you’ve collected with your letter.
Make copies, and send it by certified mail or hand-deliver the paperwork, but request a receipt. Documentation it was received is a key step to moving forward.
